Home visits

If you are housebound and need to be assessed face-to-face by one of our clinicians, then we may decide that a home visit is necessary.

Visits are usually carried out by a clinician from our visiting team, which might be a GP or one of our Advanced Practitioner team. We will only visit patients where this is absolutely necessary and when patients are unable to leave their home due to ill health or disability.

If you think you need a visit from one of our team you should submit an appointment request either online using Anima or by calling one of our sites.

If you think that you need a visit it is best if you could let us know as early in the day as possible please.

Note, home visits are not available to assist patients with transport issues.
